Follow-Up Order & Schedulers
There is an electronic “Post ED Follow-Up” order that can be accessed from the ED Quick Orders or by searching the order catalog.
This order can be used to send a request to schedulers to contact the patient and attempt to get them scheduled for a follow-up appointment.
Please note that this process does not guarantee a follow-up can be accomplished in the specified timeframe but that an attempt will be made.
Ensure that patients talk to registration before leaving, because they collect the necessary contact information for this process.
Schedulers are also available via phone at 5-2273 to help established follow-up appointments for patients before discharge.
General Approach to PCP Referral
There are several options for patients who do not have a PCP but need PCP followup. The first (which is a new option since 2022), is a UAB Post Discharge Clinic. There are a few other options outlined in the diagram further below.
UAB Post Discharge Clinic:
For followup roughly 7-14 days after discharge.
Place a “Post ED Follow-Up” order (located on the ED quick orders) and find the “UAB Post Discharge Clinic” option in the drop-down.
There is also a process whereby patients can pick an available appointment from the iPad screen used for their discharge signature (see below).
The following list includes other PCP options that can be printed for patients.
Cahaba PCP Follow-Up
Cahaba Medical Care is a potential close PCP follow-up option (24 - 48 hrs) for patients who don’t have a PCP or commercial insurance. Accepts Medicaid.
During business hours (Mon-Fri, 8a-5p), fill out the Cahaba Follow-Up Form and hand it to the OSS to schedule the appointment. The form should be pre-printed in the pods, but you can also click the link below.
Outside of business hours, provide the form to the patient and give them instructions on how to call and arrange an appointment.
UAB Family Medicine Follow-Up
UAB Family Medicine is the preferred referral option for non-urgent PCP follow up for individuals that do NOT have a PCP but do have commercial insurance.

Rabies Vaccine (Injection) Clinic
For patients needing to start the rabies vaccine protocol, there is an outpatient follow-up pathway for them to receive subsequent doses in the injection clinic.
Breast Clinic
There is a streamlined process for same or next-day follow-up to the breast clinic.
For patients with palpable masses or uncomplicated abscesses/infections

ABC Hepatitis Clinic
This is a clinic specifically for follow-up related to hepatitis.
Message “ABC Clinic Scheduling” in the message center of the patients chart. This can be found in the “pool” scheduling.
Send the diagnosis and a good contact.
Somatic Symptoms Disorder Clinic
This is a UAB clinic run by Dr. Aaron Fobian for patients with disorders such as:
Pseudoseizures
Fibromyalgia
Dysautonomia
Other functional pain syndromes.

Cancer Triage Evaluation Clinic
This is a clinic for patients with newly discovered masses that are likely cancerous and need a tissue diagnosis and workup. The idea is to prevent unnecessary admissions when the cancer workup was the only reason to admit a patient and they are otherwise stable for discharge.
Inclusion Criteria:
Patients previously not attached to Hem/Onc that are in need of evaluation of cancer.
Patients are not those who have rare disorder but rather those who are awaiting a pathologic diagnosis or those in need of testing.
Insurance type doesn’t matter.

When:
It is an APP clinic held on Fridays, 8:00-9:00am and 1:00-2:00pm
These are 1-hour appointment slots
