The ED attending of record needs to complete a “Death Note”, in addition to the usual ED Physician Note (which can be completed by the resident).

• You can find the note by searching “Death” in the Encounter Pathway tab of the Documentation (when creating a new PowerNote).

• It will have a description that says “UAB Death Note”.

• After completing this Death Note, the ED attending of record should expect an email in a few days instructing him/her to login to the Alabama EVERS website and complete the remaining elements of the death certificate.
